A 48-year-old patient applied to the dental clinic with complaints about the unsightly appearance of the upper and lower jaw teeth, and periodically arising teeth sensitivity to cold.

The main wish of the patient was to keep teeth alive, without covered teeth by crowns.

In the patient’s history: reflux ( St.localis, a mixed form of the pathological teeth abrasion; in the frontal segment - 2-3 degree, in the lateral segments - 1 degree).

All teeth except tooth 1.4 stayed vital. The tooth 1.4 was previously subject to endo-treatment, the patient planned to cover it with a crown. A dental crown is a tooth-shaped "cap" that is placed over a tooth, covering the tooth to restore its shape and size, strength, or to improve its appearance.

The patient was diagnosed with the following:

Tooth decay 1.6, 1.5, 2.5, 2.6, 3.6, 4.6.;

Pre-cervical defects of teeth 4.4, 3.2, 3.3, 3.4, 3.5, 3.6.;

Primary adentia of the tooth 2.3, on its place the tooth 6.3.;

The most pronounced tooth abrasion 1.1, 2.1, 2.2, 3.1, 4.1, 3.2, 4.2, 4.3, 3.3.;

The vertical position of the tooth 1.3.; Tremas between teeth 1.2, 1.3, 1.4, 4.3, 4.4.

The teeth abrasion in the front segment was connected not only with the dysfunction of the temporomandibular joint but also because of reflux in the anamnesis. The patient belongs to the 3rd class of occlusion. Decreasing the height of the bite by 3 mm.

After analyzing all the data and performing a functional analysis of the dental system, an occlusion splint was assigned for 3 weeks.

After getting a new position of occlusion, was started the total teeth reconstruction in a direct technique using the biomimetic method.

The work was performed using Dentsply Sirona materials: SDR, Ceram-X Duo (DB), Spectrum OA 2, Esthet X A2, A1, YE.

After the manufacture and fitting of a metal-free crown on tooth 1.4, the occlusal splint would be performed for the upper dentition: “Protection of the restoration”.
